What HSRT Questions Actually Look Like

The HSRT tests how you think, not what you know. Every question gives you a scenario and asks you to reason from the evidence — not recall facts. Here are 3 example question types:

AnalysisQuestion 1 of 3

A hospital reports that patients in Ward B have a 40% higher rate of post-surgical infections than patients in Ward A. Ward B recently switched to a new sterilization protocol, while Ward A continues using the original protocol.

Which of the following conclusions is most strongly supported by this information?

A
B
C
D
InferenceQuestion 2 of 3

A study found that nursing students who completed a critical thinking course scored 15 points higher on clinical evaluations than those who did not. However, enrollment in the course was voluntary.

What limitation of this study design most weakens the conclusion that the course caused the improvement?

A
B
C
D
NumeracyQuestion 3 of 3

A clinic serves 240 patients per week. Of these, 60% are seen for routine checkups, 25% for acute illness, and the remainder for follow-up visits. The clinic plans to add capacity for 30 more routine checkups per week.

After the expansion, what percentage of total weekly patients will be seen for routine checkups?

A

The 6 Critical Thinking Skills the HSRT Measures

The HSRT doesn't test medical facts. It tests how you reason through clinical scenarios. Each skill below is a separate dimension of your critical thinking ability.

🔍

Analysis

Identify the key elements of an argument or scenario and separate relevant from irrelevant information.

🧠

Inference

Draw conclusions that are warranted by the evidence — not more, not less than what the data supports.

⚖️

Evaluation

Assess the credibility of sources and judge the logical strength or weakness of arguments.

🔮

Induction

Form generalizations from specific observations. Make reasoned judgments under uncertainty.

📐

Deduction

Apply general principles to reach specific conclusions using formal logical structures.

📊

Numeracy

Interpret quantitative data — statistics, graphs, probabilities — and apply them to decisions.

Schools That Require the HSRT

19+ programs across Minnesota, Texas, New Jersey, and Oregon require the HSRT for admissions. If your program is on this list, you need to prepare — and there's almost nowhere else to do it.

Frequently Asked Questions

What is the HSRT exam?
The HSRT (Health Sciences Reasoning Test) is a 33–35 question critical thinking exam required by nursing and allied health programs. It tests reasoning ability — analysis, inference, evaluation, induction, deduction, and numeracy — not medical knowledge.
How is the HSRT different from the TEAS or HESI?
The TEAS and HESI test academic knowledge (science, math, reading, grammar). The HSRT tests how you think — your ability to analyze arguments, draw conclusions from evidence, and reason through scenarios. No medical facts are needed.
How long should I study for the HSRT?
Most students need 4–6 weeks of focused preparation. Critical thinking skills improve with deliberate practice but don't respond as quickly to cramming as content knowledge. Consistent daily practice (30–45 minutes) outperforms intensive short-term study.
What score do I need to pass?
Score requirements vary by program. Minnesota nursing programs typically require 60–74. Texas allied health programs vary — check with your specific program for exact requirements.
Are there any other HSRT practice tests available?
StudyBuddy offers the only dedicated, comprehensive HSRT prep available. Insight Assessment (the test publisher) offers a limited free demo through some school websites, but there are no other full practice courses.
