What are NGN questions on the NCLEX?
Next Generation NCLEX (NGN) questions are six new item types introduced to test clinical judgment, not just knowledge recall. They include Extended Multiple Response, Extended Drag-and-Drop, Cloze/Drop-Down, Enhanced Hot Spot, Matrix/Grid, and Bowtie. The April 2026 NCLEX test plan increased the emphasis on clinical judgment and unbiased, equitable care.
How many questions are on the NCLEX?
The NCLEX uses Computerized Adaptive Testing (CAT). Most candidates answer between 85 and 150 questions. The test ends when the algorithm determines with 95% confidence whether you've met the passing standard — not when you reach a fixed number. You have 5 hours.
What changed in the April 2026 NCLEX test plan?
The April 2026 update renamed the "Safety and Infection Control" subcategory, added new emphasis on health equity and unbiased care, updated substance misuse language, and removed the 1–2 minute per question pacing guidance. The client needs category weights, item count (85–150), time limit (5 hours), and NGN structure (3 case studies, 18 scored items) were unchanged.
How is the NCLEX scored?
The NCLEX uses a pass/fail system based on Item Response Theory (IRT). You pass when the algorithm is 95% confident your ability level is above the passing standard. There is no numerical score — just pass or fail. The passing standard is set by a panel of nurse educators and employers and reviewed periodically.
